D17 Auxiliary Donation Solicitation Request Form

In accordance with D17 Aux Directive 07, Flotillas Commanders may use this form to request permission to solicit gifts/donations from up to three organizations/companies. An additional form may be submitted for more organization/companies. Please fill in the form and click the Submit button below to send the request to the DCO for his consideration.
